prestonservco 26 Posted September 13, 2009 Is that a typo? Not sure what a soanks kennyme form is. For something generic, you can pick up an invoice pad from Office Max or something. lueman 19 Posted September 14, 2009 I create all my forms on MS word. I create a header graphics with my company logo & address and paste that to the header field. I use a text box to precisely set position. I then create a table field with shirt sizes from 1x to 6x and a column for style & color. In the middle I place a shirt graphic you can use a square with Length & Width and either drop in your graphic or pencil it in.
