Creating a invoice for consignment items

I'm making a few shirts to list at my local gym to test the market. At this point we're doing a consignment system.

 

Does anyone know a good template for the "invoice" and maybe some good terms and conditions (as in I own the product until it is sold and can come take it at any time etc. etc,)
