Permission sheet to use a copywritten logo

Does anybody have one?  A kids team here is going to the World Series, and they're looking for donations.  They're going to some companies for $$$ and want to put their logo on the shirts.  So, I'm looking for some type of permission slip.  THANKS!

No such thing, legally you need written permission from the trademark holder. They are getting into a sticky area there... Let them deal with getting specific written permissionfor their particular use, it isn't worth getting crosswise with some of these companies...
